How We Evaluate and Treat OCD

OCD is more than liking things neat. It is a treatable condition in which unwanted thoughts, images, or urges (obsessions) drive repetitive actions or mental rituals (compulsions) that take up time, cause distress, or get in the way of daily life. Based in College Station, Ruhwell Med provides OCD treatment in Texas by secure video and works alongside ERP-trained therapists, so your care follows the full, evidence-based approach.

Your first visit is a comprehensive psychiatric evaluation by secure video. We explore intrusive thoughts, compulsions and mental rituals, avoidance, reassurance-seeking, how much time symptoms take each day, and how they affect school, work, and relationships. We also screen for conditions that often travel with OCD, including anxiety, depression, ADHD, and tic disorders.

Exposure and response prevention (ERP), a specialized form of cognitive behavioral therapy, is a first-line treatment for OCD. Certain serotonergic medications can also ease the intensity of obsessions and the pull of compulsions, and many people do best with both. We prescribe thoughtfully, at the lowest effective dose, and pair medication with sleep, routine, and stress support.

When to Seek Help for OCD

Everyone has odd or unwanted thoughts now and then. In OCD, those thoughts feel sticky and alarming, and the urge to neutralize them with rituals, checking, or reassurance keeps growing. If symptoms take more than an hour a day, cause real distress, or lead you to avoid people, places, or tasks, it is a good time to get evaluated.

Good to know: Having an intrusive thought does not mean you want it or will act on it. In OCD, these thoughts are distressing precisely because they go against your values.

Common signs of OCD

Common Types of OCD

OCD shows up in many forms, often called themes. Most people have more than one, and themes can shift over time while the cycle of obsession and compulsion stays the same.

01

Contamination

Fear of germs, illness, or feeling unclean, leading to excessive washing, cleaning, or avoiding people and places.

02

Checking and Doubt

Persistent fear of causing harm or making a mistake, leading to repeated checking of locks, appliances, messages, or work.

03

Symmetry and Order

A strong need for things to feel even, aligned, or “just right,” with arranging, counting, or repeating until the feeling passes.

04

Intrusive Thoughts

Unwanted violent, sexual, or religious thoughts that feel deeply upsetting, often managed with mental rituals or avoidance.

05

Relationship and Moral

Constant doubt about a partner, your own feelings, or whether you are a good person, with endless analyzing and reassurance-seeking.

06

Related Conditions

Body dysmorphic disorder, hoarding, hair-pulling, skin-picking, and tic disorders are closely related. Anxiety, depression, and eating disorders also commonly occur alongside OCD.

How Is OCD Diagnosed?

There is no blood test or brain scan for OCD. Diagnosis is based on a careful clinical interview using DSM-5-TR criteria, often supported by validated rating scales such as the Y-BOCS, which also help track progress over time.

A thorough evaluation reviews:

We also look for conditions that can mimic or overlap with OCD, such as generalized anxiety, tic disorders, autism, and depression, so your plan fits the full picture. When OCD symptoms appear suddenly in a child, we coordinate any added medical evaluation with your pediatrician.

OCD in Children and Teens

OCD often begins in childhood or the teen years, and kids may not be able to explain why a ritual feels necessary. Symptoms can look like perfectionism, defiance, or meltdowns when routines are interrupted. Early, family-centered treatment makes a real difference.

In kids and teens, warning signs may include:
